The FiLaC (Fistula Laser Closure):technique of laser closure of fistula-in-ano

چکیده
The treatment of anal fistula is a real challenge in anal surgery because of a significant risk of failure or recurrence and especially of anal incontinence. Fistulotomy, in its different forms, is the treatment of choice but alternatives are sometimes necessary in cases of risk of anal continence. These alternatives, called sphincter-saving procedures are numerous, but their success rates are lower than for fistulotomy. The FiLaCTM (Laser Fistula Closure) is a new and simple technique of anal fistula treatment consisting in ‘‘burning’’ the fistula tract radially with a laser. The literature is still poor with some open and heterogeneous studies, but their results are encouraging, with a success rate ranging from 70% to 80% without negative impact on anal continence. Our own experience is going in the same direction with a 89% cure rate over the short term. More rigorous studies with a larger number of patients are needed to support these promising preliminary results of FILACTM, to compare the technique with other sphincter-saving procedures and to define its place in therapeutic strategy.

Closure of fistula-in-ano with laser--FiLaC™: an effective novel sphincter-saving procedure for complex disease.
AIM Fistula laser closure (FiLaC™) is a novel sphincter-saving procedure for the treatment of anal fistula. Primary closure of the track is achieved using laser energy emitted by a radial fibre connected to a diode laser. The energy causes shrinkage of the tissue around the radial fibre with the aim being to close the track. This pilot study was designed to investigate the safety and effectiven...
متن کاملروش جدید ترمیم زخم پس از لارنژکتومی توتال
Background: Total laryngectomy is the mainstay of treatment for laryngeal cancer. Fistula is one of the most common complications after total laryngectomy. In patients without risk factors such as prior radiotherapy, diabetes mellitus or chronic renal disease, the incidence of pharyngocutaneous fistula is related to wound healing and duration of operation. We have developed a new method that is...
متن کاملThe Application of a New Cyanoacrylate Glue in Pediatric Surgery for Fistula Closure
Surgical glues have been used in pediatric surgery because of the fragility of tissue, and to prevent major surgeries. The present report describes our experience with using a new cya-noacrylate Glubran 2 (Viareggio, Italy) in the treatment of five cases of tracheoesophageal atresia with fistula (one fistula protection, three recurrent fistula, and one unstable patients), two cases of hypospadi...
متن کاملIn-vitro Investigations of Skin Closure using Diode Laser and Protein Solder Containing Gold Nanoshells
Introduction: Laser tissue soldering is a new technique for repair of various tissues including the skin, liver, articular cartilage and nerves and is a promising alternative to suture. To overcome the problems of thermal damage to surrounding tissues and low laser penetration depth, some exogenous chromophores such as gold nanoshells, a new class of nanoparticles consisting of a dielectric cor...
متن کاملDermal island-flap anoplasty for transsphincteric fistula-in-ano: assessment of treatment failures.
PURPOSE The aim of this study was to assess the treatment failures of island-flap anoplasty for fistula-in-ano, a procedure designed to treat fistula without sphincter division. METHODS Data concerning all patients having dermal island-flap anoplasty for the treatment of transsphincteric fistula were reviewed. Variables assessed were age, gender, radial fistula location, cause, Crohn's diseas...
